By registering my child to participate in Crosstown Next, I acknowledge and agree to the following:

I am the parent/legal guardian of the child listed above and grant permission for my child to participate in Crosstown Next ministry programs and activities for children & families.

I understand that on Sundays, the ministry of Crosstown Next is provided while parents are on-site and actively attending the worship service of Crosstown Community Church.

I understand that children must be potty-trained. If a child requires additional help with toileting, the parent will be notified during the worship service.

I understand that reasonable measures will be taken to safeguard my child’s health and safety. In the event of an emergency, I authorize Crosstown Chicago ministry staff to secure any necessary medical treatment for my child.

I release Crosstown Chicago and its ministry staff from any liability for injury, illness, or property damage incurred during participation in Crosstown Next ministry activities, except in cases of gross negligence or willful misconduct.

Children must be checked in by a parent/guardian each week.

The claim tag provided at check-in must be presented at check-out for child release.
