By agreeing to the information below, I acknowledge and agree to the following:

I am the parent/legal guardian of the child listed above and grant permission for my child to participate in Crosstown Chicago Children’s Ministry programs and activities.

I understand that reasonable measures will be taken to safeguard my child’s health and safety. In the event of an emergency, I authorize Crosstown Chicago staff/volunteers to secure any necessary medical treatment for my child.

I release Crosstown Chicago, its staff, and volunteers from any liability for injury, illness, or property damage incurred during participation in Children’s Ministry activities, except in cases of gross negligence or willful misconduct.

Children must be checked in by a parent/guardian each week.

The claim tag provided at check-in must be presented at check-out for child release.
